Navigating Side Effects and Silent Challenges

At Sugar20 Clinics, we understand that a diabetes diagnosis is just the beginning of a journey. For many, the challenge isn't just managing blood sugar, but also navigating the side effects of medication and other underlying health issues. Today, we share the story of Saraswathi (name changed), a 47-year-old woman whose recent experience highlights the importance of open communication with your clinical team.


When "Standard" Treatment Feels Difficult

Saraswathi has been living with Type 2 Diabetes for two years. During her visit in February, she presented with a common but frustrating hurdle: nausea and significant abdominal complaints. Despite being physically active and walking regularly, she was struggling with disturbed sleep and nocturia (waking up twice a night to urinate).Our clinical team suspected Metformin intolerance. While Metformin is a "gold standard" for diabetes, it can sometimes cause gastrointestinal distress. Saraswathi was initially on a combination of Gliclazide and Metformin in the morning, and Metformin with Sitagliptin in the evening. When side effects like nausea become severe, they can lead to "moderate compliance," where a patient might skip doses to avoid feeling ill.


The Ripple Effect: Weight, Blood Pressure, and Anemia

By her follow-up in April, Saraswathi’s clinical picture had shifted. Her weight had increased from 61.5 kg to 64.5 kg, pushing her BMI from 25.93 to 27.20. Additionally, her blood pressure had risen from a healthy 129/83 mmHg to 144/82 mmHg.Perhaps most importantly, she was diagnosed with Iron Deficiency Anemia. Anemia can cause fatigue that is often mistaken for "diabetes burnout." Although she had been prescribed iron injections, she had not yet taken them. This highlights a key Sugar20 philosophy: managing diabetes requires addressing the whole person, including nutritional deficiencies and sleep health.


Adjusting the Sails: A Personalized Prescription

To help Saraswathi feel better while keeping her sugars in check, her physician at Sugar20 made strategic adjustments to her medication:

  • Tackling Intolerance: The morning dose was switched from a Metformin combination to Sulponyl Urea alone to reduce the load on her stomach during the first half of the day.
  • Evening Control: She continued a nighttime dose of Metformin + Vildagliptin to maintain glucose control during sleep.
  • Addressing Anemia: The focus shifted back to her Iron Ferric Carboxymaltose injections to resolve her anemia and boost her energy levels.
  • Supportive Care: She was prescribed nutritional support and Sugar-Free Supplement to ensure she receives necessary supplements without affecting her glucose levels.


The Path Forward: Data and Consistency

Saraswathi’s advice from the clinic remains clear: Brisk walking for one hour a day and, crucially, checking blood sugars every 3rd day.
